Homeless Senior Housing
On the street, older individuals (often the chronic homeless) are more susceptible to disease as a result of poor hygiene, nutrition and weather as well as at a high risk of being victims of crime or violence. Many are chronic homeless that have long dealt with alcohol, substance and mental health issues.
Working with local nursing homes, the program assisted “Senior Homeless” in obtaining permanent housing. The last quarter of 2014 demonstrates the potential of this program.

The program had placed 50 previously homeless individuals in nursing homes.

One-third of them had, while on the street, relied heavily on hospital EDs

Detox, treatment and other issues were/are being addressed

Program continues to follow-up with these clients

AA program was started to provide support and avoid relapse
Of the 50 previously homeless individuals assisted:

45 are currently housed
-
Two are back on the street (have been in and out)

Two died (One was in hospice care when referred to the BE THE CHANGE Program)

One is moving into their own home
